Provider lists can differ between plans within the same insurance company (especially between HMO & PPO plans). You may need to specify which plan you are enrolled (or reviewing) to get providers on with that particular plan.
2016 Blue Cross Medical Plans - Maricopa County Resident Notice:
HMO plans in Maricopa County offer a choice of two networks. Services outside of the network are not covered except for emergency care and other limited situations with BCBSAZ preapproval.
Alliance Network: Mostly doctors and hospitals from Banner Health and HonorHealth systems (formerly John C. Lincoln and Scottsdale Healthcare)
Select Network: Mostly doctors and hospitals from Dignity Health, IASIS Healthcare and Phoenix Children’s Hospital systems
